REGISTRATION FORM 

All priests and seminarians must send in a completed Registration Form.  If you need a PDF reader, you can download it here.

A $100.00 non-refundable deposit is required when you send in your registration form, with the remainder of the payment due upon arrival for the workshop. Those who choose to pay in full early may do so when sending in the registration form. Send your $100.00 non-refundable deposit to the "Canons Regular of St. John Cantius" and send this with your Registration Form:

Many of our workshops are held at the Stritch Retreat House on the campus of Mundelein Seminary in Mundelein, Illinois. If the workshop you are attending is held at the Stritch House, and if you wish to arrive at the Strtich House a day early, or if you wish to extend your stay after the workshop at the Stritch House, please contact Deacon Richard Hudzik, director of the Stritch Retreat House, directly to make those plans:

TRANSPORTATION TO MUNDELEIN FROM THE AIRPORT

If you are flying into Chicago and if the workshop you are attending is at the Stritch House, you may wish to fly into O'Hare International Airport, as is the closest to the Stritch House at Mundelein Seminary. Both the Stritch House and Mundelein Seminary are northwest of Chicago. It would also be feasible to fly into the Milwaukee airport to arrive at the Stritch House. If you are flying into Midway Airport, you will arrive on the south side of Chicago, which is at least 90 minutes from the Stritch House by car. If your workshop is held at St. John Cantius Church, located near downtown Chicago, know that the parish is about 30-40 minutes from both O'Hare and Midway Airports.
